Abstract
A well-done failure modes and effects analysis (FMEA) goes a long way towards designing robust products and processes. Producing one is, however, very time-consuming and can drain an organization’s resources if not planned properly. The authors propose the use of a functional matrix as one of the key tools for producing quality FMEAs. Proper use of the matrix offers many benefits. It is particularly useful when developing FMEAs for complex systems, for which functions tend to have crucial implications in performance, quality, reliability, and safety. This paper presents steps for constructing such a function matrix and linking it to reliability and validation efforts.
